Responsible to assess residents and participate in development and implementation of comprehensive plan of care. Responsible for admissions/discharges for assisted living and nursing home residents in accordance with Departments of Health/ Social Services/ Medicaid/ Medicare guidelines.

Required: Bachelor degree in social work or human services appropriate to resident needs. Minimum 3-5 years’ experience in regulatory guidelines by Virginia licensing agencies regarding Medicare/Medicaid process and transmission, but not limited to, admissions, discharges, MDS 3.0, chart documentation required. Working knowledge of medical terminology. Responsible for initial ISP and UAI documentation, to include audit and review for Assisted Living Facility.
